Welcome! We're happy to see you!

We’re glad you’ve entrusted us with your most valuable sense: your sight! We are certain this will be the most thorough eye examination you have ever received.

If you need to request an appointment for an eye exam, click the button below and send a request form or call us at 270-558-4741. If you already have an appointment, read on!

We’re excited about your upcoming exam! Did you know eye examinations are about much more than just making sure your vision is clear? We also are looking at the health of structures inside your eye which give us information about your overall health. Many diseases like high blood pressure, high cholesterol, diabetes, arthritis, and others are diagnosed after being discovered during a comprehensive eye exam!

We pay meticulous attention to details and, to help us get everything perfect, there are a few things we ask you to bring to your exam:

-Current glasses and sunglasses

-Current contact lenses

-Contact lens boxes or packaging

Patient Information

Our new patient information packet is available as an online form. This form is a secure portal that will transfer information directly into your health record in our office! We ask that every patient complete the online form prior to arriving at our office, as this saves both you and our staff time and allows the doctor more time to spend one-on-one with you.

Instructions for logging on to this secure portal will be emailed to you at the time that you make your appointment.

If, for some reason, you are unable to fill out the online form, we ask that you arrive 15 minutes prior to your scheduled exam time in order to fill out the necessary paperwork.

Additional Exam Information

Most exams last around 45 minutes, or longer if we are fitting contact lenses or if additional tests need to be performed. As part of a comprehensive examination of your eyes, you will be dilated during your visit. The drops we use for dilation have a shorter duration of action than most and typically do not drastically affect your ability to go on about your day.

If you are unable to make your appointment, we ask that you give us 24 hour notice so that the spot may be filled by another patient.

If you have any additional questions, please do not hesitate to contact us!
